The TB skin test is performed by injecting 0.1 ml of tuberculin purified protein derivative (PPD) into the inner surface of the forearm. The injection should be made with a disposable 27-gauge tuberculin syringe, with the needle bevel facing upward. The TB skin test is an intradermal injection.

A health care worker will use a small needle to inject testing fluid (called tuberculin) under the skin on the lower part of your inner arm. After the test, you should avoid using bandages, putting on lotion, or scratching the test area because it may affect the results.

Use a single-dose tuberculin syringe with a short (¼- to ½-inch) 27-gauge needle with a short bevel. Clean the top of the vial with a sterile swab. Fill the syringe with 0.1 ml tuberculin.

If you have a negative skin test, you need a repeat test at least once every four years. If you have a documented positive skin test, you must have an initial chest X-ray. After that, you still need to be screened every four years.

Side effects are uncommon from the Mantoux skin test. However, a person who has been exposed to TB germs may occasionally have a sizable reaction, which may cause some mild itching, swelling or irritation. These kinds of reactions should disappear in one to two weeks.

If you have an Mtb infection, the skin around the injection site should start to swell and harden within 48 to 72 hours . This bump, or induration, may also change color. But the induration's size, not the color, will determine your results. An induration of less than 5 millimeters (mm) is a negative result.

What if my TB skin test is negative? The test is “negative” if there is no bump (or only a very small bump) at the spot where the fluid was injected. A negative TB skin test usually means that you don't have TB. In some situations, you may need to have another TB skin test later.

Release the skin and slowly inject 0.1mL of tuberculin. You will feel a slight resistance. A firm, pale wheal 6-10 mm in diameter should appear immediately. If a lot of the fluid leaks out onto the skin or no wheal appears, administer a second injection on the other arm or at least 5-10cm (2") from the first site.

The tip of the needle should be below the fluid level in the vial. Pull back on the plunger and draw out slightly more than the one tenth of a milliliter needed for the test.
